Detection of IMP, KPC, NDM, OXA-48, and VIM genes from carbapenem non-susceptible organisms including Acinetobacter baumannii, Pseudomonas aeruginosa and members of Enterobacterales group.


NOTE:
Wisconsin Residents: Confirmed carbapenemase producer isolates will be sent to the Wisconsin State Lab for surveillance.

Michigan Residents: All carbapenem resistant isolates will be forwarded to the State Lab of Michigan for surveillance.

Collection Processing Instructions

​Perform isolation of infecting bacteria.

Bacterial organisms must be submitted in pure culture, actively growing. Do not submit mixed cultures.

Parafilm or tape the edges of the plate or slant.

Place the agar slant or other appropriate media into the secondary infectious container for shipment.

Each isolate must be submitted under a separate order.

Limitations:
The Xpert Carba-R Assay detects IMP, KPC, NDM, OXA-48, and VIM genes from pure colonies and is not for bacterial identification.

The Xpert Carba-R Assay will generate a negative IMP result when testing samples containing IMP-7, IMP-13, or IMP-14 gene sequences.

Certain bacterial species, such as Pseudomonas aeruginosa and Acinetobacter baumannii have been shown to exhibit resistance to carbapenems due to intrinsic resistance mechanisms.

The detection of other OXA-carbapenemase genes, besides blaOXA-48 and blaOXA-181, has not been evaluated.

Test Information

​The Xpert Carba-R Assay is intended to detect IMP, KPC, NDM, OXA-48, and VIM gene sequences in Enterobacterales, Acinetobacter baumannii, and Pseudomonas aeruginosa. These bacteria are often resistant to all beta-lactam agents and frequently are co-resistant to multiple classes of other antimicrobial agents.

Reference Range Information

Not Detected​

Interpretation

A detected result will be reported when one or more of the following targets is/are detected: IMP, KPC, NDM, OXA-48, and VIM. This indicates the presence of gene sequences associated with carbapenemase producing organisms.

A not detected result will be reported when IMP, KPC, NDM, OXA-48, and VIM target DNA is not detected.

A not detected Xpert Carba-R Assay result does not preclude the presence of other carbapenem-resistance mechanisms.
