Collection Processing Instructions

Place tissue in a leak-proof specimen container with 4-10 times it volume of 10% Neutral Buffered Formalin (10% NBF) and placed in a biohazard bag. Label the container with a histology request form barcode, owner and patient name. Submit specimen container with a completed histology request form containing patient demographics, history, tissue source, requesting physician and collection date. Web portal orders are accepted with a copy of the packing list or the order confirmation page included with the specimen.

One-two anatomic sources are allowed per submission. Additional charges will be applied for greater than two anatomic sources.
