Billing Policy

Clients will receive a monthly billing statement which include:

• Date of Service
• Patient's Name
• Individual Test Name
• CPT Code(s)
• Individual Test Fee

To assist you our billing office is open 8 a.m. to 5 p.m. (CST), Monday through Friday.
